Booking Policy/Late Fees
Understanding that services such as Dance Classes, Private Lessons, or Products sold by the Dance Academy cannot be rendered unless complete payment has been provided first, we ask that all dance families are using the HDA calendar or automated systems to make all payments in a timely manner.
When enrolling in the 2026-2027 Dance Year, you are committing to payments for your class for the full term August 2026-June 2027. Classes missed cannot be pro-rated based on your student's attendance, and in the case of multiple absences teachers may require you to purchase additional Private Lessons to help you catch up.
By agreeing to this policy, you acknowledge your commitment to the above, including that dancers may not book or receive classes or products without payment, and any payments incomplete after their due date will incur Late Fees after 5 days which may reoccur until the payment is complete. Students cannot book new classes etc. unless their account balance is paid in full.
Health Information and Medical Release
I, _________________, the parent/guardian of _________________________, acknowledge that participation in dance is potentially dangerous and the is an inherent risk of injury involved.
In allowing my child to participate in Ms. Hannah’s Dance Academy activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Ms. Hannah’s Dance Academy and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at Ms. Hannah’s Dance Academy.
Parent Release Form for Photography and Videography
I, the undersigned, give permission for Ms. Hannah’s Dance Academy to use video footage and / or photographs of my child/ward, _________________________________. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:
- printed materials (eg - brochures and newsletters)
- online and offline advertising and promotion
- videos and digital images such for use on Social Media.
By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Ms. Hannah’s Dance Academy for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
Water/Snack Policy
I understand that given the exertion of activities like dance and tumbling, it is very important to stay hydrated and fueled for class. In the event my dancer does not bring their own water to class or the teacher determines my dancer is feeling unwell due to lack of fuel, I give permission for his/her dance teachers to provide bottled water and/or a snack charged to my account. ($1 per item if determined necessary.)
Students or their guardian must agree to all Policies specified above before enrolling at Ms. Hannah's Dance Academy.
